L’IPAS : passe ou passe pas ?
Lorenzo Livi a presenté à San-Antonio (abstract S5-03), les résultats d’une étude randomisée comparant deux modalités de radiothérapie post-chirurgie conservatrice du sein : irradiation conventionnelle de l’ensemble du sein (50Gy en 25 séances et 5 semaines + complément de dose au lit de tumorectomie de 10Gy en 5 séances et une semaine) à une radiothérapie partielle accélérée (30 Gy en 5 séances et 10 jours).
Leurs résultats semblent satisfaisant en termes de tolérance aigüe et tardive précoce (suivi médian de 5 ans). Ils ont même trouvé qu’elle était sensiblement meilleure que la radiothérapie conventionnelle mais ceci pourrait être lié au fait que la modulation d’intensité était réservée à la modalité partielle.
Cette petite (520 patientes) étude unicentrique est surtout intéressante parce qu’elle présente montre que la radiothérapie partielle mériterait peut-être d’explorer d’autres modalités de doses que celle retrouvée des grandes études randomisées (NSABP-B39 : 3 000 patientes, RAPID : 2 135 patientes ou SHARE : 2 796 patientes) dont les premiers résultats suggèrent une augmentation significative de la toxicité 6.
Avant de pouvoir proposer à nos patientes cette modalité de traitement il faudra bien sûr avoir un recul plus important et attendre des études de confirmation.
